Job Coach - Individual Placement (Temporary, Part-time)

Summary:
The Individual Placement Job Coach is directly responsible for training and supervising all participants participating in the program at their work site in the community.
Essential Functions:
* Participates in team meetings in regards to service planning for assigned participants, in conjunction with organizational staff, caregivers, funding agencies and other persons involved with a participant’s case
* Plans, develops & implements participant-centered goals & objectives
* Trains and supervises participants on the job, on a weekly basis unless otherwise instructed
* Ensures participant work performance meets employer’s standards
* Maintains professional relationship with employer, and receives participant updates form the employer on a weekly basis
* Completes any documentation and support paperwork in an accurate and timely manner
* Participates in required meetings and trainings
* Assists and provides direct service to participants and support to agency staff in implementing service plans and behavior strategies, monitoring goal progress, identifying additional service needs, and addressing other rehabilitation issues
* Assists in the development of individualized training programs
* Maintains necessary records, evaluations and billing, and submits all in a timely manner.
* Maintains effective communication and acts as a liaison between the consumer, GGRC, Department of Rehabilitation, and all other involved parties
* Ensures a safe continuous working environment according to general safety practices
* Reports any participant or employer problems that might arise to the dept Coordinator or Department Manager
* Must comply with applicable policies, i.e., Community Gatepath Safety Policies, CARF accreditation standards, CAL-OSHA, etc.
* Comply with participant rights, Community Gatepath program policies and procedures, and quality assurance standards
* Has independent and reliable transportation to and from Job sites
* Able to occasionally work nights and weekends as needed
* Other duties as assigned
Physical Activity Requirements:
* Ability to sit and keyboard – frequently
* Ability to bend, squat and stoop – frequently
* Ability to lift 25 lbs. – occasionally
* Ability to physically respond to physically frail persons - occasionally
Education/Experience:
* Knowledge of computers including Windows, Excel and Word
* Ability to work with detailed and confidential information
* Positive outlook, enthusiasm, self-motivated and high energy.
* Ability to function as part of a team
* Proficient oral and written communication skills
Licensing Requirements:
* Valid California Driver’s License/ID
* Proof of negative TB test results
* Verification of good physical health by physician
* CPR Certification/First Aide Training
* Ability to obtain criminal fingerprint clearance
* 18 years of age
Training Requirements:
* Required to complete a minimum of ten (10) hours of training or continuing education as directed and approved by your Supervisor
